Nose Aesthetics (Rhinoplasty)
Rhinoplasty operations, changing the shape of the nasal arch or tip; It is performed to reduce or enlarge the size of the nose. In addition, the size of the nostrils, the area between the root of the nose and the forehead, or the angle between the tip of the nose and the lip can be improved. Problems such as polyps, sinusitis, nasal concha and deviated septum that prevent the fulfillment of respiratory function can also be eliminated.
During aesthetic nose surgery, respiratory function should be observed and a nose shape should be made in accordance with facial harmony. In order for the surgical factors not to be noticed, natural lines should be adhered to. What kind of nose should be created after the operation is decided by the patient and the doctor together. The nose shape preferred by the patient should be suitable for his/her gender, age and culture.
Why is rhinoplasty necessary?
In 90% of the nose surgeries we perform, there are deformities and breathing complaints. Many people want to change shape while their breathing problem is healed. This is the right decision; because the cartilage tissues removed in the nasal surgery called septoplasty are used for reconstruction for aesthetic purposes. Since the necessary cartilage tissues cannot be found in intranasal and aesthetic operations performed at different times, it is necessary to take these structures from the ear or ribs.
One of the most frequently asked questions by the patient's family and close circle is whether the nose needs aesthetics. The ideal answer for this is; If the person is uncomfortable with the shape of the nose while taking a photo or looking in the mirror, surgery can be performed.
Aesthetic nose surgery is performed on people with nasal deformity and breathing difficulties due to deformity. Not every deformity in the nose is associated with breathing problems. However, the low tip of the nose often causes breathing problems and the patient is not aware of it. In this respect, a detailed nasal examination is required before the operation. In addition to nasal examination, intranasal examination with endoscopic method is also required. It should be evaluated whether the nasal conchae are large and whether there is a nasal sinusitis. If sinusitis surgery is required for enlargement of the nasal flesh that will prevent breathing, as well as deformity that will disturb the person himself, all surgeries can be performed at the same time, together with aesthetic nose surgery.
Who is plastic nose surgery rhinoplasty for?
Aesthetic nose surgery can be performed on people with nasal deformities and respiratory problems. Some breathing problems are caused by existing deformities. Low nose tip is one of them. Therefore, a detailed nasal examination and endoscopic intranasal examination are also required before surgery. The size of the concha and the presence of sinusitis-related discharge in the nose should be evaluated. If the patient has both deformity and breathing problems, all procedures can be performed simultaneously with aesthetic nose surgery.
Except for nasal deformities due to congenital or subsequent injury; It is recommended that both men and women have completed the age of 17 for the full development of the spiritual, psychosocial and anatomical structure.
What kind of nose should I get?
How the nose should be depends entirely on individual factors. If the nose shape desired by the patient is in a way that will impair the function, the physician should be warned. However; Since the nose desired by the person and the shape of the nose understood by the physician may be completely different, the procedures to be performed should definitely be discussed in great detail. Since the changes made on the photos of the nose are two-dimensional, the change photos of other patients can also be checked in order to understand all the changes.
As a result of a detailed examination and a joint decision, the patient and the physician should be in absolute consensus before the operation on the shape of the nose. Although the different nose shapes shown by the patients in the media give clues about what the person wants to tell, no suggestion is made to make the same nose. The face is a whole and the nose should be evaluated together with the face. A beautiful nose on another face may not be suitable for another face.
How is the evaluation done before rhinoplasty?
In the interviews held before the operation, first of all, the patient's complaints and expectations about the current nose are evaluated. The nasal cartilage structures of the nose, whose external structure is examined, are felt by palpation. If there is a low nasal tip, the level is evaluated by pressing with the finger. The patient is breathed normally and forcefully, and the degree of collapse of the nasal lateral walls is determined.
Photographs of the nose are taken from different angles, and the possible post-operative image is created on the computer. A common opinion is reached by showing the photographs of other patients who have previously had aesthetic nose surgery close to the same nose shape as our patient.
If there is a breathing problem, intranasal structures are checked in detail with the help of an endoscopic device. In more severe cases, sinus film should be taken; The presence of problems such as polyps, sinusitis, nasal concha and deviated septum should be detected. Both deformities and structural problems can be healed during the same operation. When all the examinations are completed, the patient's blood tests are completed and the surgery day is planned after the anesthesia team approves the patient.
Do I have a chance to see the shape of my new nose before the surgery?
The opinions of both the patient and the doctor are very important to determine and understand the shape of the new nose. The nose image to be performed after the surgery is shown to the patient with the help of photoshop, and the shape of the nose can be discussed. Again, the patient is helped to have detailed information about the result of the operation by showing the real results of the surgeries of a similar nature.
How to prepare before rhinoplasty operation?
However, if the drugs need to be used continuously, the prescribing doctor should be consulted first. It is recommended not to smoke for a while before and after the surgery. There is no harm in the operation for women to be in the menstrual period.
Before coming to the hospital on the day of the operation, make-up should not be applied, products such as hair gel should not be used, jewelry should not be worn and comfortable clothes should be worn. No food or drink should be consumed 6 hours before the surgery. Being at the hospital about 1 hour before the appointed time will make the procedures easier.
Which anesthesia is used in rhinoplasty surgery?
Aesthetic nose surgeries are performed under general anesthesia. Local anesthesia or sedation can be used in surgeries involving only the tip of the nose. In operations performed under general anesthesia, the patient's blood pressure and pulse can be adjusted as desired, blood loss is less. New generation anesthetic agents have been developed considerably. The patient's recovery from the effect of anesthesia is more comfortable.
How is aesthetic nose surgery performed?
Aesthetic nose surgeries may vary depending on the area to be treated and the needs. There are many types of surgery and which one to choose is determined as a result of planning. Open or closed technique is used as the surgical technique. In the open technique, an inverted V-shaped incision is made on the skin of the structure called the columella, located in the middle of the nostrils. It is preferred in revision, traumatic and broken nose surgery in cases where the tip of the nose needs to be healed. In the closed technique, no incision is made on the skin, the procedure is carried out by making an incision through the nose. If the patient does not have a deformity at the tip of the nose, only if there is a protrusion in the nasal bone, the closed technique is applied.
In most of the nose surgeries performed with open or closed technique, the reconstruction of the nose, called reconstruction, is performed. Excess cartilage removed from the nose in the operation is used to strengthen the nose and allow the patient to breathe comfortably.
Depending on the procedure and the size of the problem, the duration of the operation varies between 2-3 hours.
Closed technique or open technique?
The major factor in deciding the appropriate technique is the nose structure of the patient. If there is no functional problem or excessive deformities, the closed method will be the first choice. The open technique is suitable if it is necessary to correct major problems or to reshape the nose. Which method is required is specified during the preliminary interviews and examination by the physician.
Open technique may be preferred in surgeries where the tip of the nose needs to be corrected, and closed technique may be preferred if only the nasal arch is to be repaired. The choice of closed or open technique also depends on the surgeon's predisposition. While some surgeons can perform all operations with the open technique, some surgeons prefer only the closed technique. There are also surgeons who successfully apply both techniques.
In the closed method, the structures removed from the nose are visible, and the remaining structures are estimated. The remaining structures are more important than the structures removed in aesthetic nose surgeries. Even if the open technique is preferred, the incision in the structures at the tip of the nose heals quickly and after a few months, the scars disappear.
During open aesthetics, an incision is made in the area between the nostrils and the nasal skin is taken up. It is an advantage especially for surgeons who are in the first stage of their first training years, as the entire internal structure can be visualized. Surgeons, who frequently apply this technique during their education years, continue with the method they are accustomed to. In the open technique, which is easier to learn and apply compared to the closed technique, there is a very low probability of scarring.
Closed rhinoplasty (rhinoplasty) technique is generally used in aesthetic nose surgeries. The cartilage, bone and soft tissue that form the basis of the nose structure is reshaped. The low, excessive height at the tip of the nose, the angle of the nose and the lip, the protruding or missing area in the nasal bridge, the curvature of the nose, the abnormal structure of the nostrils are corrected and they look aesthetically pleasing. However, deviated septum (curvature of the middle wall of the nose) and conchae (nasal meatus), which are mainly effective in breathing problems, can also be intervened. Special tapes that facilitate healing are adhered to the outside of the reshaped nose, and a splint plaster is placed, which will remain for about 1 week.
The common concern of many patients who want to have rhinoplasty is whether there will be a scar after the surgery. Although aesthetic operations have been performed for years, thanks to technological changes and scientific innovations, scars are not formed after today's surgeries.
How is the recovery process after nose surgery?
Due to the development of rhinoplasty techniques in recent years, comfort can be provided both during and after surgery. There is very little bleeding during the surgery, and minimal redness and swelling afterwards. Most of the patients stay in the hospital for one night and are discharged on the second day, depending on the doctor's decision, after 5-6 hours.
He does not need to use tampons after his surgery, depending on his own technique. Silicon perforated nasal pads, which are required to be put on after the surgery of some patients, are removed after one night, and breathing can be done at night with these pads. It is not necessary to apply tampons in rhinoplasty. However, the use of tampons provides an advantage to the patient as less swelling is seen.
A plaster cast should remain on the back of the nose for a week; One week later, plaster and sutures should be removed and a thin plaster should be applied for three days. Skin-colored bands are important for the resolution of edema in the nose. Patients can start their normal office work in three days and their daily work in about a week. Walking or light exercises after 3 weeks are not objectionable; however, it is necessary to wait 2 months for heavy sports and exercises and 6 weeks for swimming.
Some numbness at the tip of the nose is considered normal. This problem disappears within 2-3 months. In order to facilitate healing and eliminate edema, it is shown how to do a nose massage depending on the structure of the nose. In addition, our patient is explained in detail what to pay attention to after the operation by the staff of Antalya mediface medical center.
The new shape of the nose can be observed as of the 3rd week. However, it may take between 6 months and 1 year for the nose to take its final shape. Studies have shown that patients who are satisfied with their new appearance have positive effects on human relations and behaviors.
